Purpose. The worksheet the HACCP team uses to analyse hazards at each process step, decide critical control points and record limits, monitoring and corrective actions.

1.Purpose

This template is used when a new product or process is introduced, when an existing process changes and at the annual review of the HACCP plan. It records the hazard analysis for every process step from receipt to dispatch and the decisions that follow from it.

2.Listing process steps

Use the verified flow diagram for the process. List every step in order, including storage, transfer and rework steps. Each step is analysed separately.

3.Identifying hazards

For each step, identify the biological, chemical, physical and allergen hazards that are reasonably likely to occur. Record the source of the hazard and the evidence for it, such as complaint history, supplier data or scientific literature.

4.Assessing significance

Rate the likelihood and the severity of each hazard using the risk matrix in the HACCP plan. Record the control measure that prevents, eliminates or reduces the hazard, and apply the decision tree to decide whether the step is a critical control point.

5.Critical control points

For every critical control point record the critical limit, what is monitored, how, how often and by whom, the corrective action if the limit is exceeded, the verification activity and the record kept. These entries transfer to the HACCP plan summary.

6.Sign off

The HACCP team leader and every team member present must sign the worksheet. The worksheet is filed with the HACCP plan and superseded versions are retained.
